Birth and death records can be obtained by calling (216) 529-7690 Monday through Friday from 9:00 a.m. to 4:00 p.m.
Funeral directors are encouraged to call for an appointment.
A fillable PDF application to request a certified birth or death certificate is available and can be mailed, faxed, or e-mailed to the Division. Please note that a required fee of $25.00 is charged for every copy of birth and death records certified. Further information is below.
REQUESTING BIRTH AND DEATH RECORDS:
The Division can make a certified copy of a birth certificate of any citizen who was born in Ohio. The Division can also make a certified copy of a death certificate of any citizen who died in the City of Lakewood.
To obtain these certificates, download our fillable application online and return the completed copy by mail, e-mail, or fax. NOTE: You MUST open the application in Adobe Reader for the fill-in fields to work properly.

Fees:
In accordance with section 3705.24 of the Ohio Revised Code, we are required by law to charge a fee for each certified copy of a vital record issued. The fee at this office for each certified copy of a birth, death, or fetal death record is $25.00.
Please submit payment with your application by mail using a money order or bank check made out to the City of Lakewood—personal checks will not be accepted. If you are requesting a birth or death record by fax or email, our staff will contact you for payment before processing the request.
